LIABILITY WAIVER
I.
I, the undersigned, being aware of my own health and physical condition, and having knowledge that my participation in any strenuous fitness program may be injurious to my health, have voluntarily submitted myself in the participation of any physical activity available to me by your establishment.
II.
I further agree to disclose any physical limitation, ailment, disability or impairment, which may affect my ability to participate in any fitness program.
III.
Fit Crew Inc (Corner Crew/Corner Crew Cycling Club) reserves the right to refuse service to any patron, who, upon disclosure of any limitation, ailment, disability or impairment, may not be fit to engage in strenuous physical activity, except in instances where a doctor’s recommendation and/or clearance have been approved.
IV.
Thus, I hereby waive and release the company, any of its representatives, agents, and successors-in-interest from liability for any accidental injury, illness or death which may incur during and/or as a result of my participation in any physical activity organised by the company within or outside the premises of the establishment.
V.
I understand and agree that Fit Crew Inc is not responsible for property that is lost, stolen, or damaged while in, on, or about its establishment.
